Comprehensive Plan

$ 395 per year
  • 2 exams per year
  • Routine X-Rays
  • 2 basic hygiene cleanings
  • 15% off emergency exams
  • 15% off all restorative treatment
  • $550 off Clear Aligner Orthodontic Treatment
  • 30% off preventive specific services (fluoride, sealants, cavity arresting medicine, etc.)
  • * Add a family member and receive 5% off each additional plan.

Periodontal Disease Plan

$ 525 per year
  • Includes everything in the Comprehensive Plan, plus...
  • 1 additional periodontal maintenance cleaning for a total of 3 maintenance cleanings a year (1 every 4 months following completion of SRP)
  • 25% off scaling and root planing treatment (deep cleaning)
  • 25% off of Arestin medication
  • Estimated value: $1,116

Program Guidelines

  • Our membership plan is for uninsured patients of our practice and is not an insurance plan.
  • Pricing is based on a one-year contract and is non-refundable.
  • All plan benefits must be used within 12 months of activation.
  • The plan renews automatically on the anniversary date, with the full membership fee due on that date, unless a member provides notice to terminate at least 30 days before renewal.
  • Your doctor will complete a comprehensive examination and diagnose the care needed to support your health.
  • HSA and FSA payments are accepted.
